2018/19 Max in Motion Player Application Announcement

Arizona Soccer Association (ASA) is pleased to announce that we have been selected as a participant in theMaxInMotion® Foundation for the seventh year. MaxInMotion®, over the past six years has provided ASA Members more than $750,000 in funding! MaxInMotion® is a benefit for ASA members, with every penny fromMaxInMotion® going directly to clubs to support our players. The administrative cost to administer the program is absorbed by ASA in order to maximize the benefit to our members!

About MaxInMotion® Foundation: MaxInMotion® provides much needed financial assistance to youth sports programs for underprivileged children and for the development of programs to include special needs populations in youth sports activities. Research shows that participation in youth sports enhances community values, interdependency, and collective responsibility. Physical fitness prevents childhood obesity and has also been linked to reducing school dropout rates, teen pregnancy and high-risk behaviors, all of which pose tremendous consequences to our society. MaxInMotion® has developed partnerships with other youth sports organizations, including Coyotes Charities, the Arizona Diamondbacks Foundation, Phoenix Suns Charities and the Arizona Bridge to Independent Living (ABIL).MaxInMotion® was founded in memory of Max Shacknai, who embodied the ideal young athlete: passionate, hardworking, and incredibly committed to his teammates. Max Shacknai was the inspiration for MaxInMotion®. Max was a young, talented athlete who tragically lost his life in July 2011. His love of sports, soccer in particular was known throughout the youth sports community. Inspired by his energy, spirit and commitment, MaxInMotion® is a not-for-profit foundation dedicated to providing young athletes an opportunity to compete at both the recreational and competitive levels in major youth sports.

To begin the Process:

·Contact your club to ensure they are participating in the program.

·Complete the online application.

·Submit all required documents to your Club.

The deadline for submitting applications and required documents to ASA is Monday, July 31, 2018 at 4:00pm Arizona time. Please check with your Club for their deadline, as they will need time to prepare all documents prior to submission to ASA. Applications and documentation must be submitted by the Club only - NO EXCEPTIONS.

Requirements for this application include:

  • 2018 MaxInMotion Player Application
  • 2017Parent/s W2/1099
  • 2017 Parent/s Federal Full Tax Return, including all schedules
  • Most recent Player report card(Player must carry a minimum of 3.0 GPA in the most recent report card to be eligible for an award)
  • Listing of Community Service performed in the last year
  •  

Incomplete applications will not be considered.

We plan to have our recommendations for awards to MaxInMotion® by mid August and we are hopeful to have monies by the end of September.
