Foundation to financially help those in need with the costs of participating in youth soccer

     Phoenix, Ariz. — June 7, 2016 – AYSA is pleased to announce that we have been selected as a recipient of the MaxInMotion® Foundation for the fifth year in a row!

In celebration of our fifth year of participation, AYSA is excited to announce MaxInMotion® as the Presenting Sponsor of the 2017 Arizona State Cup Championships!

Max Shacknai was the inspiration for MaxInMotion®.  Max was a young, talented athlete who tragically lost his life in July 2011.  The first word out of Max’s mouth as a baby was “Ball!”  His love of sports – soccer in particular – was known throughout the youth sports community.  Inspired by his energy, spirit and commitment, MaxInMotion® is a not-for-profit foundation dedicated to providing young athletes an opportunity to compete at both the recreational and competitive levels in major youth sports.

The Foundation provides financial support directly to youth sports organizations throughout the state of Arizona to help underwrite inclusion in sports programs of financially challenged boys and girls who would not otherwise be able to participate.  The Foundation embraces the philosophy that youth sports contribute dramatically to character development and personal fitness, and that team participation helps establish values of community, interdependency and collective responsibility.  To learn more about MaxInMotion®, please visit

For those families who have participated in the past, you are probably aware that in the first year, we received 164 applications.  In year four, we received 1,083 applications!  We hope to exceed that number this year.

Using the same format as last year, families must submit their application online.

Player Applications:

  1. Beginning June 8, 2016, complete your application online using the following link:
  2. Submit your required documents (w2’s, Federal Tax Returns along with all related schedules) to your Club.
  3. Clubs must submit all required documents to the AYSA office no later than 4:00pm on Wednesday, July 6, 2016

**Please note, State Tax forms are not required this year. However, we will not accept State Tax Returns in place of Federal Tax Returns.

Once completed, submit your required documents to your club.  Each participating Club will then be responsible for submitting all documents to AYSA.  The deadline for club submissions is 4:00pm on Wednesday, July 6, 2016.  Your club may have an earlier deadline for submitting your Player Application, please contact them for verification.

Very Important:

Clubs have been instructed to only submit completed applications.  This includes W-2 forms and Federal tax return/all related schedules. If you are self-employed, Federal tax returns with all related schedules will be required and likewise, submitted to your club.  Please do not submit incomplete applications as they will be declined.

MaxInMotion® provides much needed financial assistance to youth sports programs for underprivileged children and for the development of programs to include special needs populations in youth sports activities.   Research shows that participation in youth sports enhances community values, interdependency, and collective responsibility.  Physical fitness prevents childhood obesity and has also been linked to reducing school dropout rates, teen pregnancy and high-risk behaviors, all of which pose tremendous consequences to our society.
